Practice Policies

Appointment Policy
Appointments are available Monday through Friday from 8:00 AM to 1:00 PM and can be made by calling (561) 318-3727. Arrangements can be made on a different schedule on certain occasions. An initial consultation will range from 1/2 to 1 hour, depending on the complexity of your case and number of conditions that need to be addressed.

Appointment Cancellation Policy
Since Dr. Porrata schedules her patients so there is minimal wait time and your appointment is not rushed, we exclusively reserve your appointment time to discuss all your concerns. In order to best serve all of our patients, we will accept cancellations if given 24 hours in advance and will gladly reschedule you for another time. Except for emergencies, if there is less than a 24 hour notice for a cancellation, a cancellation fee of 50% of the value of the scheduled visit will be charged.

Phone Messages and Communication
Your telephone calls are always welcome. We do not have an answering system company. All phone calls and messages are directly received by your doctor or office manager and will be answered promptly. We will do our best to try to answer your concerns as soon as possible during our working day, but patients being seen at the office take priority. Non urgent messages left after business hours (8am-3pm) will be returned the next business day. If you have a medical emergency, please call 911 or go to the nearest emergency room.

Medication/ Refills
Medication changes are made at appointments only unless authorized by your provider. Medication prescriptions that are given at the time of the appointment are intended to last the patient through until their next scheduled appointment. Refills for regular, not controlled substances will be processed as non-emergency. If the request is received from the pharmacy or a phone message from the patient, we will take up to 48hrs to process. We will not process refills after business hours (8am-3pm).

 If you are prescribed a controlled substance, state regulations are followed; patients need to be present at the time of appointment for any changes or refills. Dr. Porrata does not prescribe narcotic pain medication or refills controlled substances prescribed by other physicians you might be seeing. If you lose your prescription or medication bottle for a controlled substance (appetite suppressants/benzodiazepines) this must be reported to the police. A copy of the police report must be provided with your refill request in order for it to be processed.

Billing and Payment
You will be expected to pay for services at the time of each session. If your account has payment overdue for over 60 days, our office will make a courtesy call to make you aware of this balance. The payment must be made in full before another appointment can be scheduled. If at 90 days, your account is still overdue you will receive written notice of collection activity and your care will be terminated.

Patients may receive bills in the mail, which may be paid by check, or calling the office with a credit card number. If there’s a discrepancy between what the bill states and what the patient believes he/she owes, the patient must call the office as quickly as possible to resolve this issue.

Consents
Care will not be initiated until all proper consents are signed.

Insurance
The only item in our service not covered by insurance are the office visit fees. All Diagnostic tests and most medications are covered by your insurance carrier. We do not participate in any insurance programs. However, at your request, we can provide you with documentation that includes appropriate diagnosis codes and shows that you have paid in full. This documentation can be submitted to your insurance company. If your policy has out-of-network benefits you will be reimbursed accordingly. Dr. Porrata will provide you with a prescription for any blood work or testing so that you can take this to a lab or testing facility that is in your insurance network, same case with any other diagnostic testing needed or referral for specialist consults. If you are currently or have recently been seen by a primary care physician or specialist, we will request your prior records. This will allow us to avoid duplication of diagnostic tests. We will also provide you with all the documentation needed to submit for your insurance carrier or life insurance carrier to adjust their premiums once your risk factors and weight have improved. This type of intervention is also tax deductible and you should submit it for your CPA to review.

Confidentiality
We adhere to HIPAA laws and procedures. All medical information is protected information and will not be released unless consent is signed by the patient. It is understood that all information between patient and provider is held strictly confidential and will not be released unless permitted by law.

Termination of Care
Patient treatment may end when either treatment goals have not been met, or when the patient and/or doctor find it necessary to do so. A patient may terminate treatment at any time, for any reason. If patient has not received care for more than 12 month, he/she is considered to be self-terminated and their file is closed. If patient has not received care in person at an office visit for more than 6 month, we will not refill medications and a new visit will be required to review plan of care and appropriate prescriptions. The doctor may decide to end care with a patient for reasons included but not limited to dishonest or inappropriate behavior towards the clinical or administrative staff, missed appointments, nonpayment of bills, or the inability of treatment goals to be met with outpatient care. If the doctor decides to terminate treatment with a patient, the patient will be notified of this decision verbally or in writing. 

In many cases Obesity Care is also tax deductible and you should submit it for your CPA to review.

Medicare patients

We have applied to be a Medicare provider, as soon as we are approved we will communicate this to our patients currently enrolled in the Medicare program and we will be billing Medicare for services provided.
