No annual membership is required, as well as no initial financial commitments or prepaid packages. Fees are due at the time of the appointment only. You will be charged only for the scheduled visits.

Preventive Medicine | Personalized Care Visits

We offer 4 levels of appointments determined by the time of the visit. Your provider will determine the minimum level appropriate for your care. Visits are charged by time and consist of 15 minute intervals.

Standard Rate for Appointments:

      • Simple (15 minutes):           $125.00
      • Moderate (30 minutes):       $250.00
      • Extended (45 minutes):       $375.00
      • Complex (60 minutes):        $500.00
      • Dispensed Medication in office or supplements: you will be informed of the total at the time of check out.

Bariatric Medicine | Lifestyle Intervention

Individual Visits (one patient)*:

      • 1st Visit: $185 Includes Review of Medical History, Behavioral and Nutrition Assessment, Request for Appropriate Diagnostic Tests.
      • 2nd Visit: $135 Physical Exam, Review Test Results, Comprehensive Plan  Nutrition: Behavioral and Exercise plan. Referral for Specialist Consults if indicated.

 * $100 will be added on increments of half hour past the hour visit, to both individual visits and couples visits.

Couples Visits*: 40 minutes to 80 minutes Depending on Complexity

      • 1st Visit: $325 this will include Review of Medical History, Behavioral and Nutrition Assessment, Request for Appropriate Diagnostic Tests.*
      • 2nd Visit: $250 Physical Exam, Test Results Review, Comprehensive Nutrition, Behavioral and Exercise Planning. Referral for Specialist Consults if indicated.*
      • Follow-Up Visits: $250 Review of Medical, Behavioral and Nutrition Progress.*

* $125 will be added on increments of half hour past the hour visit, to both individual visits and couples visits.

IV Therapy:
$150 - $175 per session. All patients will require an initial consultation (please see Preventive Medicine for rate schedule). Follow up consults will be scheduled on an as needed basis.

We will arrange a first introductory visit to groups of three or more. The cost for the visit will be $150 per person. In this visit we will review the overall plan and the nutrition plan in general. Visits will have to continue as individual visits from then on as our plan is individually tailored and the Patient-Doctor relationship is confidential. Individual fees will apply.

Phone Consultations
Phone Consultations are available on certain occasions at the same rate of an office visit. These are consultations with  Dr. Porrata, they are  scheduled during regular office hours and will take the place of an office visit. For this reason these consultations are treated as regular visits.

After hour calls are calls received outside of the normal office hours (8:00 AM - 5:00 PM; Monday - Friday). These phone consultations will be answered by Dr. Porrata and will be charged at the following rates of:

Monday- Friday:           $50.00
Saturday- Sunday:       $85.00

*These are not intended to replace a regular office visit.

Return Policy: There are no refunds on opened supplements. We will gladly offer an exchange or a refund on any unopened supplements. Prescribed medications cannot be returned.

New Patients
Dr. Porrata is currently accepting new patients.

Appointment Policy
Appointments are available Monday through Friday from 8:00 AM to 1:00 PM and can be made by calling (561) 318-3727. Arrangements can be made on a different schedule on certain occasions. An initial consultation will range from 1/2 to 1 hour, depending on the complexity of your case and number of conditions that need to be addressed.

Appointment Cancellation Policy
Since Dr. Porrata schedules her patients so there is minimal wait time and your appointment is not rushed, we exclusively reserve your appointment time to discuss all your concerns. In order to best serve all of our patients, we will accept cancellations if given 24 hours in advance and will gladly reschedule you for another time. Except for emergencies, if there is less than a 24 hour notice for a cancellation, a cancellation fee of 50% of the value of the scheduled visit will be charged.

Phone Messages and Communication
Your telephone calls are always welcome. We do not have an answering system company. All phone calls and messages are directly received by your doctor or office manager and will be answered promptly. We will do our best to try to answer your concerns as soon as possible during our working day, but patients being seen at the office take priority. Non urgent messages left after business hours (8am-3pm) will be returned the next business day. If you have a medical emergency, please call 911 or go to the nearest emergency room.

Medication/ Refills
Medication changes are made at appointments only unless authorized by your provider. Medication prescriptions that are given at the time of the appointment are intended to last the patient through until their next scheduled appointment. Refills for regular, not controlled substances will be processed as non-emergency. If the request is received from the pharmacy or a phone message from the patient, we will take up to 48hrs to process. We will not process refills after business hours (8am-3pm).

 If you are prescribed a controlled substance, state regulations are followed; patients need to be present at the time of appointment for any changes or refills. Dr. Porrata does not prescribe narcotic pain medication or refills controlled substances prescribed by other physicians you might be seeing. If you lose your prescription or medication bottle for a controlled substance (appetite suppressants/benzodiazepines) this must be reported to the police. A copy of the police report must be provided with your refill request in order for it to be processed.
